  • One in five Californians faces food insecurity. Food insecurity is linked to diabetes, heart disease, and other chronic conditions, and costs the healthcare system $5 billion annually.
  • More than one-third (35%) of Californians needing home-based support couldn’t access it due to financial constraints, lack of information, or unavailability in their community.
  • Transportation barriers are often a cause of missed appointments among seniors.

Our team of dedicated social workers:

  • Provide education and guidance, in applying for:
    • Medi-Cal
    • Medicare
    • In-home and community-based supportive care [In Home Support Services (IHSS), Community Based Adult Services (CBAS), Multipurpose Senior Services Program (MSSP)] for Medi-Cal recipients
    • CalFresh (food assistance programs for low-income residents)
    • ACCESS transportation services
    • Short-term disability (EDD) benefits
  • Provide linkage to homeless services and supports
  • Educate and connect members and their families to caregiver resources and respite support
  • Promote understanding of, and connection to, palliative or hospice care, as appropriate
  • Supporting members and their families in understanding options related to advance care planning and the process for completing:
    • Advance healthcare directives (AHCD)
    • Physician Orders for Life-Sustaining Treatment (POLST)
    • Durable power of attorney (DPOA)
  • Assess for fall risk and offer tools to reduce the risk of falls that can lead to injury
 
 

How to refer members to the Social Worker team

For members and providers in both the North and South:

  1. Complete the Social Work Referral Form.
  2. Email Regal North completed form to SWReferralNorth@RegalMed.com. You can also fax the completed form to (818) 540-3258.
  3. Email Regal South completed form to SWReferralSouth@RegalMed.com. You can also fax the completed form to (818) 540-3243.
  4. For San Fernando Valley, Antelope Valley, Simi Valley, and Ventura County, email the form to RegalNWSocialWorkReferral@RegalMed.com. You can also fax the completed form to (818) 540-3258.
  5. Once received, our social work specialist will review the referral. Accepted referrals will be assigned to a social worker within 1 business day. If there are any concerns with the referral, our coordinator will contact the provider for any further clarification.

Note: You do not have to submit an authorization in REA for members to receive social work assistance from the Social Worker team.

Members can also self-refer directly by reaching out to the social worker team to request assistance in finding resources and services relevant to their specific needs.
